Christina was able to avoid further patient harm by performing excellent chart reviewing and delivering excellent communication to all staff involved. This patient was previously WBAT, then had surgery and was supposed to be NWB. However, provider notes did not reflect the change in WB status, nor did the activity order in the chart. Christina was able to do an extended chart review and communicate directly with ortho and RN on this error. This action was able to correct all incorrect documentation and improve patient care.
