Health Information Management (HIM) would like you to ask yourself four questions when you are scanning:
- Is the correct Patient and Encounter selected before scanning the image/document?
- Are the patient labels applied? If the document is double sided, are both sides labeled and scanned (even if one side is blank)?
- Is the Note Type and Subject Title correct and titled?
- Last but not least, is the Quality and the Alignment of the scan legible?
